Hospital Facilitator

This is an opportunity for an ambitious Hospital Facilitator to join an established care company. We are looking for dedicated and empathic people. Working in the healthcare sector can be a challenging environment, but the sense of achievement and daily recognition more than compensate.  The position is 30 hours per week

Reporting to the Lead Facilitator, the Hospital Facilitator is responsible for the support with the promotion of a timely, safe and appropriate discharge from acute inpatient care to the community by way of the Allied Healthcare Reablement Service.

As the Hospital Facilitator your responsibilities will include:
• To participate in the clinical discussion of management of a designated group of service users.  
• To work as a member of the MDT in assessing and planning care, responding to changes in service users’ health.  
• To identify eligibility for meeting reablement service criteria and share best practice with other health professionals.
• To liaise with Care Coordinators, Ward Teams, Hospital at Home Teams, Support & Recovery workers and other agencies. These could include, Housing Officers as part of discharge facilitation.

You must possess a minimum of NVQ Level 2 in Health and Social Care (NVQ Level 3 highly desirable). You must demonstrate commitment to working within a team and have a flexible approach to working hours and practices. Experience of working within a care environment for 12 months or more is essential.
