Senior Care Coordinator

CRG Homecare is looking for an enthusiastic, dedicated Senior Care Coordinator to join our team covering areas across Hammersmith and Fulham.

What you will be doing

As the Senior Care Coordinator you will be responsible for the operation management of the care team and managing the day-to-day operation of the Homecare service within the registered area, in a competent and professional manner, ensuring compliance with Local Authority contracts and CQC outcomes. Ensuring the management of the weekly services are delivered in line with their purchase order/private contract and that staff receive regular supervision as defined by the Company or best practice guidelines as defined by the Regulator.

What the role involves:

  • Approve new customer set ups, customer change in provision and termination of care and ensuring appropriate funder approval and payment.
  • Ensuring Carer rotas are constructed and prepared in a timely manner, travel time. All double up packages to have a double up run completed and care workers to be arriving and leaving all visits at the same time.
  • Managing Carer and customer compatibility and authorising exclusions and the de-allocation of Carers from visits
  • Managing risk, Health & Safety within the team including reporting on incidents, accidents and acting as an escalation point in emergencies.
  • Leading and encouraging continuous improvement activity, reviewing targets, performance and Branch Improvement plan with the Branch Manager on a weekly basis.
  • Reviewing Team Management information and holding daily team huddles and weekly performance review meetings with Care Coordinators
  • To define special care needs based upon identification of specific clinical problems, and to monitor and review as appropriate.
  • To liaise as needed with external organisations (CPN’s, Chiropodists, GPs and other nursing professionals) in respect of developing a total package of care for a Service User, and ensuring that they are made aware of all changes in the Service Users condition pertinent to their condition.
  • To keep abreast of all new developments in care and of the philosophies and policies of the contracting authority, and to communicate the same to the staff of the organisation, modifying policies as needed.

What you will need:

  • Passionate and driven to provide excellent operational support and guidance to the company.
  • Analytical and problem solving in their approach exchanging ideas and working out potential solutions to a range of issues.
  • Able to demonstrate at least 2 year’s experience of working in a ‘hands on’ care operational role.
