Enablement Facilitator

Reporting to the Branch Manager, the Enablement Facilitator is responsible to work with the Care Co-ordinator to develop an individual service user’s outcome focused support plan into their enablement delivery plan. This enablement delivery plan will support GHC to deliver an effective service which supports people to develop or re-develop skills to live as independently as possible within their local community. This will include support to improve both their health and well-being.

To be responsible for the effective deployment of a team of enablers, within a geographical area, to ensure the individual needs of the service users are met as per the enablement delivery plan.


Role & Responsibilities
•    To take appropriate action in respect of all new referrals to the enablement service from Adult Care Service or the Emergency Duty Team including some rapid response.
•    To undertake an initial enablement visit with each new service user who is referred to the enablement service, in order to write the appropriate enablement delivery plan and risk assessment.
•    To develop the enablement delivery plan with the service user, their carers and enablers, using the outcomes drawn up by the Care Coordinator, to ensure that the individual’s needs are met throughout the enablement process. This will first be updated during the 2 day follow up meeting with the service user.
•    To deploy a team of enablers to work with service users ensuring that hours of work and continuity of service are provided so that the key objectives and outcomes from the enablement delivery plan are met. This includes organising effective working patterns for enablers in order to maximise utilisation of resources.
•    The ability to work effectively with professional staff such as Occupational Therapists and Physiotherapists in order to contribute to interventions and therapy planning were necessary.
•    To be responsible for ensuring that service delivery is maintained during any period of staff absence, such as sickness, annual leave and training.
•    To undertake the supervision and appraisal of enablers in accordance with all relevant legislation and policy.
•    To have the ability to rapidly increase or decrease the amount of hours an individual receives from the enablement service, in discussion with the enablers and the Care Coordinator, in order to ensure that the individual’s needs are continually met.
•    To review the delivery of enablement for individuals throughout the process, initiating progress meetings as required with the Care Coordinator.
•    Contribute to the monitoring and evaluation of the enablement delivery plan reporting back issues or changes to the Care Coordinator.
•    Maintain accurate records including financial and staffing administration.
•    To organise, attend and contribute to meetings and supervision sessions as required.
•    To participate in all relevant learning and development, adopting your own learning attitude as well as supporting enablers with their development through shadowing and mentoring.
•    To ensure an observance and adherence to all relevant legislation and company policies and procedures.
