Extra Care Senior Care and Support Worker - Crewkerne


Reporting to the Field Care Delivery Manager the Extra Care Senior Care and Support Worker is responsible for ensuring that the daily living needs of the customers of the Extra Care House Scheme are met in partnership with the housing provider. They will need to provide a skilled and sensitive service to support the care and support workers in the provision of day-to-day personal assistance, emotional care, physical care and the promotion of the customers’ independence.  
Role & Responsibilities
•    To enable Customers to live in their own homes and in a manner that reflects their personal preferences and interests.
•    To provide support, encouragement and practical assistance which helps the Customer to maintain and promote independence, which may include planned risk taking.   
•    To communicate back to the Field Care Delivery Manager any relevant concerns or issues you may have in connection with the Customer.
•    To comply with Company policies and procedures.
•    To observe good practice (as set out in Handbooks, Selection Training, Training sessions and advice).
•    To conduct yourself professionally at all times, this includes complying with the uniform and work wear policy (unless otherwise specifically exempt) and displaying a photographic identity badge, being a reliable time-keeper, remembering that you are a guest in the Customers home. To promote a clean, positive and facing culture.
•    To ensure that no tasks are completed unless specified in the Customer Care Plan and any changes made to the Care Plan are reported back to the Line Managers.
•    To inform the Company of any warning, reprimand, caution, conviction or any other known substantial information which occurs, during and after your recruitment, selection training, training and any engagement (work) that you undertake whilst working through the Company.  
•    To support carers, advocates, health professionals and other care organisations in effecting the most coordinated and seamless package of care possible.
•    To maintain accurate records held in the tenant’s home by completing the care and support plan and other relevant paperwork in full.
•    To maintain and audit medication (MARS) records and care worker visit log books in line with the company requirements
•    To attend weekly meetings with the Housing Scheme Manager
•    To attend training as and when you are required and be committed to undertaking advanced training to enable you to perform the role.
 
 
•    To ensure that all timesheets and other required paperwork is collected and returned to the office, completed correctly and within any deadlines specified.
•    To undertake risk assessments and consult with the Field Care Delivery Manager /CDM/ Monitoring officer regarding concerns or necessary action required.
•    To respond to emergencies within the ECH scheme you are working in within the appropriate timeframe as directed by the contract.
•    To promote the professional image of Allied Healthcare with all customers, their families and other professionals.
•    To ensure confidentiality is maintained at all times by both yourself and other colleagues
•    Any other duties which the Field Care Delivery Manager / CDM / Monitoring officer considers necessary to support and ensure that services provided to the customers / tenants are maintained.
Personal work plan to achieve objectives:
•    To be agreed by the immediate Line Manager with the employee as individual cases arise, with the employee’s progress being reviewed at regular pre-determined intervals. Specific duties will be specified in the Care Plan.
 
Performance evaluation to be measured through:
•    Client satisfaction (as determined by informal and formal feedback)
•    Weekly liaison with your immediate Manager
•    Case meetings
•    Regular supervisory sessions including yearly performance reviews
•    Selection Training and ongoing personal development
Resources available:
•    In addition to support from local office(s) and central administration, specialist resources may be accessed through the Company database or register as required on a case-by-case basis.

•    NOTE:  AT NO TIME MUST A SENIOR OR CARER UNDERTAKE NURSING TASKS

Qualifications and Experience
•    No experience required (subject to contract requirement)
•    Formal selection training upon engagement, with additional training as required by legislation, good practice, service development and/or identified personal development needs.
•    QCF level 2 or above or working towards
•    All Senior Care & Support Workers are required to undertake professional QCF/SCQCF qualifications. (Qualification Credit Framework / Scottish Qualification Credit Framework). This includes registration of and commencement of the appropriate QCF level training in care within 6 months, and completion of this qualification within three years.
•    All Care & Support Workers to undertake update training on an annual basis
•    The duties listed above are not exhaustive.  Additional hours of work may be required, as determined by the needs of the business.  Managers and staff may be required to undertake additional duties, responsibilities and projects as appropriate, and in a professional manner.
