Position: Field Care Supervisor / Care Co-ordinator
Branch: Southampton
Location: Portsmouth with travel to Southampton once per week
Role: Permanent position working 37.5hrs per week
Salary: Upto 19,000 per annum

The Field Care Supervisor / Care Co-ordinator is responsible for the effective management and supervision of Care workers and supporting the Branch Manager to ensure the branch is providing an excellent provision of service to our customers, clients and colleagues on a daily basis. Ensuring compliance with national minimum standards and using effective rostering to fully utilise staff hours and drive employee engagement.  You will be required to provide day-to-day support and assisting with the assessment, set up and review of health and social care packages.      

Your responsibilities will include:
•    Ensuring that the client receives the appropriate care package and that continuity of care is a priority
•    Providing telephone support to clients and care staff and where necessary, to work as a “hands on” carer in setting up the packages and delivering care to our clients
•    To assist in auditing the standard of service delivery to clients
•    To conduct Care worker interviews & their compliance
•    Ensure that the care team are fully trained in all areas
•    Complete the Risk Assessments and care plans of all domiciliary packages
•    Allocation of care workers to customer visits by using effective rostering, ensuring continuity of care and taking into account locations, skills required and customer preferences.
•    Take responsibility for new referrals from private customers and social services teams, ensuring full information is received and appropriate care planning is undertaken.
•    To identify further business opportunities outside of current client base.
•    Matching care workers to customers, ensuring care workers are notified of all conditions relating to the assignment, including care worker’s rate of pay.

Qualifications, Experience and Requirements
•    You must possess a minimum of NVQ Level 2 in Health and Social Care
•    You must demonstrate commitment to working alone and within a team and have a flexible approach to working hours and practices.  
•    This role will involve an element of on-call duties, therefore a driver with access to your own vehicle is essential.  
•    This role will suit someone experienced in the Domiciliary care environment, who has ideas and able to grow the business within the Portsmouth area, someone who can lead the best team and who is looking to progress to a Senior/Manager role
