Acute Facilitator

Due to a successful tender, a exciting opportunity has developed for an Acute Facilitator to work on our Reablement Service based from our branch in Essex and working with the Reablement team.

As an Acute Facilitator you will be an ambassador for the Allied Health Care brand, identifying areas of potential growth and advising internal & external stakeholders on the services we can offer to our customers. You will support a safe and appropriate discharge from acute inpatient care back to the community through the Allied Health Care Reablement Service by regularly reviewing discharge care plans with our Customers, Care Team and the Multi-disciplinary team to ensure objectives are completed.

The successful candidate will have excellent people and communication skills, have experience or be trained in the ability to provide clinical supervision and will have a minimum of a NVQ L2 or equivalent experience.
